Sadguru Sri Madhusudan Sai continues the mission of love and service established and led by Bhagavan Sri Sathya Sai Baba by example. Following in the footsteps of Bhagavan, Sri Madhusudan Sai plays a key role in furthering Baba's mission by establishing various humanitarian service projects in 35 countries around the world.

Since 2011, around the world, he guides and leads dedicated volunteers and devotees by example in establishing institutions of excellence in the fields of education, health, nutrition, and spirituality. 

To date, Sadguru Sri Madhusudan Sai has established four specialized pediatric cardiology hospital centers, a multidisciplinary secondary care hospital center, two exclusive mother and child hospital centers, 27 educational campuses, a Veda gurukulam for the promotion of vedic studies learning and research, and a nutrition program that provides daily meals to over 500,000 school children from impoverished backgrounds. 

Construction is also underway for the first University Hospital in Muddenahhalli, India, where all patient services and educational processes for students will be completely free. 

Throughout the world, Sri Madhusudan Sai has established twelve Centers for Human Development based on spiritual, humanitarian, educational, and social activities.
